Supervising Diplomate Forms

The Certification Process Flow Chart will provide guidance regarding forms and payments required of Supervising Diplomates and Residents. Please also refer to the form lists and links below.

Applications for Prospective Residents
Please reference the Credentials Information page for more information. The Provisional Resident applications are handled by the ABVO® Credentials Committee.

Residency Evaluations
Evaluations are to be conducted via an online system. Supervising Diplomates and ABVO® Residents must submit forms through the online portal. If you are having difficulty signing into the portal, please contact the ABVO® office. Relevant details will be posted on this prior to the January 5th due date. Questions may be directed to the Residency Committee.

Residency Training Program Application form
Download and complete this Word document (updated 07.27.22) and send it to the ABVO Office.
Complete RTP Applications from institutions that are currently training one or more Residents must be submitted by AUGUST 15TH for consideration prior to the ACVO meeting or FEBRUARY 15TH for consideration in the spring. Programs will not be considered on a rolling basis. It is the goal of the Residency Committee to review and approve/decline all RTP applications by October 1st and April 1st. A delay may occur if additional information is requested by the committee. Individual requests (last minute funding, etc.) may be considered on a case to case basis as long as existing programs are submitted 90 days before the start of the program and new programs are submitted 120 days before the start of the program. Please direct any questions to the Residency Committee.

Changes were made to residency requirements, which resulted in the modification of this form in September 2017, read a message from the ABVO and Residency Committee which highlights these changes.

Supervising Diplomates and External Participant Residency Information Form
In order to facilitate timely tracking of Supervising Diplomates’ (Sup Dip) information for residents, the ABVO will require all Supervising Diplomates to submit the “Supervising Diplomate” form, beginning August 15th, 2017.

Supervising Diplomates are required to submit ABVO notification form when the following occur:

  1. Becoming a Supervising Diplomate or External Participant for the first time, for each resident.

  2. After a time lapse, if changing or adding additional residents whom are overseen by that SD or EP.

  3. If exiting from a current residency program and will be replaced by another Supervising Diplomate.

IMPORTANT! Before completing this form to remove your status as an SD or EP, you must submit the “Certifying Examination Supervising Diplomate Form" for credentials. This is available when signed in to your ABVO® portal, located directly under the ‘Supervising Diplomate Forms’ tab. If this is not submitted through the portal first, you will lose all access to all SD forms on the portal and not be able to complete these for residents in the future.

Submission and confirmation received via the Supervising Diplomate form are the only official means of communication to the Residency Committee in this matter. Emails to the office and to the Residency about such changes will no longer be accepted.

Instructions to Residents

Residency Committee Requirements Update:

The Residency committee has met and determined the following in regards to residency requirements.

  1. Surgical and case minimums will be suspended for residency programs ending in 2021

  2. Surgical and case minimums will be revisited for subsequent resident classes at the annual ACVO meeting based on the data provided by the 6 month evaluations.

  3. Clinical week minimums (104 weeks total over a program) will be continued and programs are advised to monitor their resident’s clinic weeks to ensure these are met if the hospital has been closed or the resident ill for COVID 19.

  4. If hospital closure and/or resident illness adversely impact the ability of the program to adjust the schedule to ensure minimum clinical weeks are met, the ABVO Residency Committee will consider the impact of the deficiency on a case-by-case basis. It is possible that such a deficiency may require extension of the residency

With this in mind, please continue to log all surgeries and case numbers on your google sheet as you have previously. The data will be crucial in determining the impact of the COVID 19 shutdown on residency productivity.

Online Reimbursement Requests

For Committees and Resident Course Reimbursements
(Updated 4.22.2019)

The ACVO and ABVO provide for an online submission of expenses toward official meetings and activities. Board and committee members may submit for reimbursement using the below summary spreadsheet and online form.

Additionally, this form is to be used by residents submitting for reimbursement of up to $500 for their registration in the Basic Science Course or the ACVO Vision for Animals Phaco Course.

Steps to submit for reimbursement online using a digital form:

  1. Complete this summary expense spreadsheet (in Excel)

  2. Complete this digital 'reimbursement form'

  3. Attach the expense sheet and any digitally scanned receipts to the digital 'reimbursement form'

  4. Submit the form, with attachments (backup of receipts, course registration proof, etc.).

Only complete requests will be processed, allow 2-3 weeks for payment receipt via check in US funds. If a bank wire is required, please email the office.
